Have a Prescription

Prescriptions are not required for any of our products. However, to bill insurance for products like the OraStretch press and TheraBite jaw motion rehab systems, or the TheraPacer CPM, a prescription is required for coverage. See the bottom of the page for more information if you do not already have a prescription.

Already have a prescription?
There are four ways we can obtain a copy to use to bill insurance.
1. Fax the prescription to us.
2. Email a scan or picture of the prescription to us.
3. We can request a copy of the prescription from your doctor.
4. Mail us the prescription.

By FAX: Send us the prescription by fax to 303-480-9115.
By EMAIL: Email a scan or photo of the prescription to support (at) craniorehab.com.

Be sure to INCLUDE:

The prescription;
your name, address, phone #, and date of birth,
your doctors name and phone #;
your insurance information (a copy of your card front and back is good).

Please call our office to confirm receipt of the prescription at 1-800-206-8381.

Request from Provider:
Complete the following form to have us request a copy of the prescription from your provider.

We will request a copy of the prescription and your insurance info from your provider. This process can take a couple days, and we will contact you to confirm the request.

We will work with you, your doctor and your insurance company to provide this equipment to you. After receiving the prescription, we will contact you insurer to confirm your coverage and benefits. We may need to request more information from your doctor, including diagnosis codes and their NPI number.
